Persyaratan klinis meliputi sebagai berikut: Indikasi yang tepat, duplikasi obat, alergi, dan reaksi obat yang tidak diinginkan (ROTD), kontraindikasi, dan interaksi obat. Apoteker memeriksa resep untuk mengidentifikasi potensi kesalahan pengobatan dan bekerja untuk menghindarinya. Medication error yang dapat mempengaruhi pasien akibat penggunaan obat ketika berhadapan dengan tenaga kesehatan yang dapat dihindari (Hindratni et al. 2017). Tujuan penelitian untuk mengetahui gambaran dan tingkat keparahan interaksi obat yang terjadi saat obat diabetes mellitus digunakan di rumah sakit saat ini. Penelitian ini menggunakan metodologi penelitian deskriptif dan desain penelitian retrospektif, yaitu penelitian dengan mengambil data -data yang telah lalu. Resep yang dikumpulkan diambil dari bulan Oktober-Desember 2022. Sebanyak 500 resep yang terkumpul merupakan salah satu resep yang diterima di Instalasi Farmasi Rumah Sakit Advent antara Oktober-Desember 2022. Kriteria inklusi untuk skrining resep polifarmasi klinis di Rumah Sakit Advent untuk pasien diabetes mellitus tipe II adalah 131 resep dari total sampel 500 resep. Dari 131 resep Ketepatan indikasi, dosis obat, aturan, cara, dan lama penggunaan obat sudah terpenuhi 100%. Pada resep yang berjumlah 3 obat terdapat (25%) yang obatnya berinteraksi. Resep yang berisi 4 obat terdapat (72%) yang obatnya berinteraksi. Resep yang berisi 5 obat terdapat (65,21%) yang obatnya berinteraksi. Resep yang berisi 6 obat terdapat (60,71%) yang obatnya berinteraksi. Resep yang berisi 7 obat terdapat (52,94%) yang obatnya berinteraksi. Resep yang berisi 8 obat terdapat (58,82%) yang obatnya berinteraksi. Resep yang berisi 9 obat terdapat (63,63%) yang obatnya berinteraksi. Resep yang berisi 10 obat terdapat (66,66%) yang obatnya berinteraksi. Resep yang berisi 11 obat terdapat (100%) yang obatnya berinteraksi

ABSTRACT
Clinical requirements include the following: Appropriate indications, drug duplication, allergy, and adverse drug reactions (ROTD), contraindications, and drug interactions. Pharmacists check prescriptions to identify potential medication errors and work to avoid them. Medication errors that can affect patients due to drug use when dealing with health workers can be avoided (Hindratni et al. 2017). The purpose of this study was to determine the description and severity of drug interactions that occur when diabetes mellitus drugs are currently used in hospitals. This study uses a descriptive research methodology and a retrospective research design, namely research by taking past data. The prescriptions collected were taken from October-December 2022. A total of 500 prescriptions collected were among the prescriptions received at the Adventist Hospital Pharmacy Installation between October-December 2022. Inclusion criteria for screening clinical polypharmacy prescriptions at Adventist Hospitals for patients with type 2 diabetes mellitus II is 131 recipes from a total sample of 500 recipes. Of the 131 prescriptions, the accuracy of indications, drug dosages, rules, methods, and duration of drug use have been fulfilled 100%. In the prescription of 3 drugs, there were (25%) that the drugs interacted. There were prescriptions containing 4 drugs (72%) whose drugs interacted. There were prescriptions containing 5 drugs (65.21%) whose drugs interacted. There were prescriptions containing 6 drugs (60.71%) whose drugs interacted.
