ABSTRACT
Background: Problems that commonly occur in outpatient services in hospitals are the length of waiting time, administrative processes, availability of information, and others. One of the quality improvement methods used in this research is lean management with the aim of achieving the effectiveness and efficiency of the use of time, manpower, space, and other resources. This study aims to describe the value stream mapping (VSM), analyze what becomes waste, and propose improvements using a lean hospital approach in outpatient services at Metta Medika II Sibolga Hospital.
Method: The research was conducted with a qualitative approach through direct observation, in-depth interviews and document review. The research subjects were six informants. registration officer, cashier clerk, polyclinic nurse, medical record officer, pharmacy officer and head of outpatient unit.
Results: The results showed that the highest Non Value Added value was in BPJS patient services with supporting examinations. The waste that occurs is in the form of waste of waiting, transportation, overproduction, overprocessing, inventory, motion, human potential and defects. From the results of the fishbone diagram, it is known that the root causes of problems that occur in the polyclinic unit are in terms of management, equipment, layout, officers and information systems. Suggestions for improvement that can be given are in the form of short-term, medium-term and long-term proposals.
